COVID-19 VACCINATION APPOINTMENT RESERVATION VIA ECHANNELLING - FAQ

1. WHAT IS COVID 19 VACCINATION APPOINTMENT RESERVATION

The customers can reserve an appointment via eChannelling service to get Covid 19 Vaccination

2. TO WHOM THIS SERVICE IS ELIGIBLE?

The service is eligible only to the residents of the Colombo Municipal Council (CMC) area (Colombo 1-15 only)

3. HOW CAN CUSTOMER OBTAIN A TIME RESERVATION FOR COVID 19 VACCINATION?

The service is available via following options

  • eChannelling website (www.echannelling.com)
  • eChannelling mobile application
4. WHAT IS THE AGE LIMIT FOR THE APPLICANT TO OBTAIN THE COVID 19 VACCINATION?

The customer/applicant must be over 60 years to obtain the COVID-19 vaccine.

(At the moment this is the age limit given by CMC. The age limit is subjected to change with the instructions of CMC in future)

5. WHAT ARE THE INFORMATION REQUIRED TO BOOK TIME SLOT FOR COVID 19 VACCINATION?
  • Applicant’s full name
  • Valid National Identity Card (NIC) Number (This should be the person who wish to get the Vaccination )
  • Applicant’s Residential Address
  • Applicant’s Phone number
6. HOW DOES THE APPLICANT/ CUSTOMER KNOW ABOUT CONFIRMATION OF THE APPOINTMENT?
  • Once the applicant makes the appointment and confirms the details, he/she will receive a One Time Password (OTP) for the given mobile number.
  • The applicant should then confirm the OTP to receive the final confirmation receipt of the appointment.
7. WHAT DOCUMENTS NEED TO BE PROVIDED WHEN APPLICANT/CUSTOMER VISITTO CENTRE IN ORDER TO GET THE VACCINATION?
  • The applicant’s Original National Identity Card (Compulsory) for identity verification purposes.
  • If the address mentioned in the NIC does not match with the applicant’s address in the Colombo Municipal Council area provided when securing the appointment, the applicant should provide evidence such as billing proof, an extract of the voter registry to prove that he/she is a resident in the Colombo Municipal Council area.
8. WHAT IS THE PROCESS OF OBTAINING THE VACCINATION AFTER GETTING THE CONFIRMATION SMS VIA ECHANNELLING?

The applicant/customer needs to produce the SMS confirmation from eChannelling to the relevant center officer of Colombo Municipal Council before getting the vaccination.

9. WHAT ARE THE BOOKING CHARGES?

The service is provided free of charge

11. ARE THESE CENTRES AVAILABLE WHEN MAKING THE RESERVATION?

Yes,
The applicant/customer can select their preferred center/location when making the reservation/appointment via eChannelling system

12. CAN THE APPLICANT/CUSTOMER’S APPOINTMENT DATE BE CHANGED?
  • Colombo Municipal Council reserves the right to cancel or re-assign the appointment to a date and time of customer/applicant preference in the event Colombo Municipal Council is unable to provide you the COVID-19 vaccine on the given appointment.
  • The customer will receive the date and the time of the new appointment via an SMS or by telephone call from Colombo Municipal Council.
13. CAN THE APPLICANT/CUSTOMER CANCEL THE APPOINTMENT AND APPLY FOR ANOTHER DAY?

No
Applicant/Customers are responsible to attend to their appointment on-time and ensure to fix their appointment to a date and time convenient for them.

14. WHAT IF CUSTOMER MAKE A MISTAKE WHEN MAKING AN APPOINTEMNET VIA ECHANNELLING?

Neither CMC nor eChannelling shall be responsible for any errors that customer make whilst using the eChannelling service.

15. IN ANY CASE OF INQUIRY WHAT IS THE CONTACT NUMBER GIVEN FOR CUSTOMER/APPLICANT?

By accessing and using the eChannelling system to obtain an appointment number for COVID-19 vaccination you hereby unequivocally accept and agree to be bound by the terms and conditions set out below. IF YOU DO NOT AGREE WITH ALL OF THE TERMS AND CONDITIONS HEREIN, YOU SHOULD NOT ACCESS AND/OR USE THE ECHANNELLING SYSTEM TO OBTAIN AN APPOINTMENT NUMBER FOR COVID 19 VACCINATION.

  1. This COVID-19 vaccination is applicable to the residents of the Colombo Municipal Council area only.
  2. The applicant must be over 30 years to obtain the COVID-19 vaccine.
  3. The person who wishes to obtain COVID-19 vaccine must provide accurate details as requested on the eChannelling system including National Identity Card (NIC) number and phone number.
  4. Once the applicant makes the appointment and confirms the details, he/she will receive a One Time Password (OTP) for the given mobile number. The applicant should then confirm the OTP to receive the final confirmation receipt of the appointment.
  5. When arriving for the vaccination for the preferred center, the applicant must produce the Original National Identity Card (Compulsory) for identity verification purposes. If the address mentioned in the NIC does not match with the applicant’s address in the Colombo Municipal Council area provided when securing the appointment, the applicant should provide evidence such as billing proof, an extract of the voter registry to prove that he/she is a resident in the Colombo Municipal Council area.
  6. Appointments made on this eChannelling system are not transferable and no proxies shall be allowed. You must attend the COVID-19 vaccination in person.
  7. There shall be no charge/fee for the COVID-19 vaccination.
  8. Colombo Municipal Council and eChannelling do not guarantee that every person who obtains a time slot through the eChannelling system will be able to obtain the vaccine on the given date/time.
  9. Colombo Municipal Council reserves the right to cancel or re-assign your appointment to a date and time of your preference in the event Colombo Municipal Council is unable to provide you the COVID-19 vaccine on the given appointment. You will receive the date and the time of the new appointment via an SMS or by telephone call from Colombo Municipal Council.
  10. You are responsible to attend your COVID-19 vaccination appointment on-time and ensure to fix your appointment to a date and time convenient for you
  11. Colombo Municipal Council and eChannelling shall only be responsible for appointment time slot reservation service.
  12. Neither Colombo Municipal Council nor eChannelling shall be responsible for any errors you have made whilst using the eChannelling service.
  13. To ensure efficient service, you are required to produce the SMS confirmation to the relevant center officer of Colombo Municipal Council which you have received from eChannelling confirming your appointment.
  14. eChannelling’s services are limited to making an appointment with the Colombo Municipal Council area based on the information you provide on the system.
  15. You consent to eChannelling processing any personally identifiable information you provide on this system for the purpose of making an appointment with Colombo Municipal Council and for any incidental purpose thereto.
  16. Notwithstanding anything to the contrary, eChannelling and Colombo Municipal Council shall not be liable to the applicant and/or to any third party for any direct, indirect, consequential, special, incidental and/or punitive damages, lost profit, lost revenue, mental distress, inconvenience, hardship, loss/corruption or destruction of data and/or the like arising from the use of the eChannelling reservation service and/or arising from the applicant’s decision to obtain the COVID-19 vaccine and/or applicant’s inability to obtain the COVID-19 vaccination on the given appointment. eChannelling is not responsible for any death or injury that may arise from use of the COVID-19 vaccination. eChannelling’s liability is excluded to the maximum extent permitted by law.
  17. eChannelling shall not be responsible for any appointments not honored by applicant and/or Colombo Municipal Council. Re-scheduling of any appointments should be directly dealt with the Colombo Municipal Council.
  18. Colombo Municipal Council and eChannelling have the right to revise the terms and conditions herein from time to time without notice to you and you shall be bound by all such revisions made by the Colombo Municipal Council and eChannelling from time to time. It is your responsibility to periodically review the latest version of the terms and conditions uploaded on the eChannelling website to stay informed of updated and revisions.
  19. Colombo Municipal Council and eChannelling reserve the right to terminate this reservation service and/or COVID-19 vaccination project at any time.
  20. These terms and conditions are governed by the laws of Sri Lanka and subject to the exclusive jurisdiction of the courts of Sri Lanka.
